  Case intake is performed through a combined system of mail intake and walk-in applications. Potential clients must complete an intake application and attest to be a US citizen. Applications are reviewed each week and assigned to staff for follow-up by providing a rejection notice, an advice only letter, request for more information, or request for appointment letter to the applicant. An applicant with an emergency can have his/her case expedited by coming to the office to fill out the necessary forms. Applications are available at any of our five offices during regular business hours. One may contact any office by phone to request an application by mail. Applications must be returned to the appropriate office. Other intake opportunities: Bonifay Holmes County Courthouse and Senior - Call for appointment (850) 769-3581. Chipley Washington County Courthouse and Senior Center - Call for appointment (850) 769-3581. Panama City Domestic Violence Center and Shelter - Call for appointment (850) 769-3581.
